Primary symptoms

  • Main complaint in one sentence (e.g. right-sided headache).
  • When symptoms started, sudden or gradual.
  • Frequency: daily, weekly, or only when triggered.
  • Factors that worsen and that relieve the symptoms.
  • Whether symptoms interfere with work, sleep, or daily activities.

History & timeline

  • Similar episodes in the past, when, and how severe.
  • Chronic conditions: hypertension, diabetes, heart, thyroid, autoimmune.
  • Past surgery, head injury, or hospital admissions.
  • Family history of similar neurological concerns.

Medication & supplement list

  • Drug name, dose, schedule, and how long it has been used.
  • Medications used as needed (pain relief, sleep aids, herbal products).
  • Drug allergies and the reactions that occurred.
  • Photos of medication packaging if names are hard to recall.

Test results & referrals

  • Referral letter from a previous doctor, if any.
  • Recent MRI, CT, EEG, EMG, or blood test results.
  • Discharge summary or notes from the last consultation.
  • Blood-pressure, blood-sugar, or sleep logs if relevant.

Questions for the doctor

  • What are the likely causes of my symptoms?
  • Which tests should be considered and why?
  • Initial treatment options and expected re-evaluation timing.
  • Signs that warrant earlier follow-up contact.

Visit logistics

  • ID, insurance card, and emergency contact.
  • A companion if the patient needs help with communication or mobility.
  • Written notes with the main symptoms and questions.
  • If relevant, short videos of tremor, seizure, or gait change.
